TeleHealth Operational System of Records (THOR)

This portal is an extension of THOR and serves as a repository of all Clinical Discovery records created by the CPM team. 

Clinical Discovery forms are regularly shared with site users. These forms capture several attributes of site and contact personnel at the site. Once the information is available from site, the data is automatically updated in THOR. These details are important for CPM group in locating site information and maintain current data in THOR.

Guide to fill the form

1. Site users will receive form links for their respective sites via an automated email from svcbuildtegria@providencetelehealth.com
2. For new implementation, the form must be filled out as comprehensively as possible to include all relevant data.
3. In case of a review, the site user must ensure that the data is accurate and up to date.
4. Site users must reach out to their Clinical Program Manager for further clarifications.
